Perhaps you should also take your console so that conditions can be exactly reproduced. Sounds like media (possibly sh*t in a small groove) to me because that particular copy is at the centre of the problem, but you can't rule out the PS2 or its laser.
Try washing the DVD in warm soapy water to get any grooved dirt out. Cheers
