Name the game "CRASH BASH - USA" (Exactly like this but no quotes)

Turn the cheats on and exit. You still have to use any original(or demo) CD when turning your PSX on. Then swap with your Crash Bash copy and 'Start game with cheats'.

Before entering the codes, I got an error message at the start of bootup saying the software is being terminated, etc..but now it works with the cheat codes.

I burned the original Crash Bash CD using CloneCD v. 2.3.3.3
I have PSX 7501 w/ an MP3 Smart DJ enhancer card (it plays MP3's too)

Hope this helps some of you with the game enhancer cards. Not sure if it will work with everything. Good luck.
