How NOCD cracks are made?

I'm sure most of us just download our nocd cracks from gamecopyworld and the like lol but I haven't really seen anyone explain how its done.

I've made a few trainers for games, nothing special but I learned most of how its done from helpful people with tutorials. So I was hoping someone might be interested in breaking down the process for making your own nocd crack?

How's it done? What tools needed? Lets say the game is protected by TryMedia + ActiveMARK v6 for steam downloads and maybe SecuROM v7 for retail. Are their different approaches based on how the game is protected? I choose these only because I see them used a lot lately, and knowing how to defeat some windows 98 game protected by DuckTape 3 isn't going to help in today's gaming

Thanks for whatever info you can provide, I do not expect it to be something simple or every idiot could do it. I just want a head start on how its done, if someone is interested in making a walkthrough on making one for a recent game that would be great. I'm sure that new COD game will need one, maybe someone would like to fraps their work and do a step by step or something?

OllyDBG or Softice with plugins are used.
Experts code their own tools.
Every protection is different to crack.
New Cod will have steam, very easy compared to activemark or securom 7.
