Ok...
There is a problem that have been quite often to me and to more people!
The "Damage or Dirty Disc" error
What my expirience says is that, till the moment... "nobody" really understood the reason of that problem. At least, here.
On this forum, some people, trying to help, alreay said that "maybe is the Xbox driver that is not working well" or "the HDD of the Xbox..." But none of this reason are right, because the same game, depending of the backup, works or not!
Some people said that coud be the dashboard, but I red in this forum that some pleople made some changes to it and did not work. Also, a friend just have the "original" dashboad and the proble continues...
Some people also told to change the media for quality one... I already used : TDK, Ridisc, Verbatin, Intenso, ... and the problem continues !
So, I thought that one possible reason was the program that creates the ISO file. My "last" bad experience was with Fable and PSI Ops (on the last one was not possible to see the "movies", but the game worked), so I changed the program (was using xISO and also tried Craxtion 2.x and CloneXB 2.3) and with Craxtion 4, I was able to make working backups :) . But now with Mercenaries, the problem "rise" AGAIN  (I used also CloneXB, but the result was the same).
So... Please... Do somebody really knows what to do to avoid that error or how to make the ISO files correctly :)
(I already made a few working ones, but just in some NEW games the problem is happening)

I know this not want you want to hear.......But is is almost 100% the dvd drive......I know sometimes some games work and then others do not....No matter what media you use it happens and then does not happen......Try and swap the drive out with another and I gurantee it works. You say that BRAND NEW games also do it...Then it is the drive....I replaced 5 drives last week and they were usually Old Thompson drives and then some of them were new Phillips drive.....The more you play it the more it will wear it down.....At least in my vast experience....hehe

HTH and Good luck

Eric
